What is Community Care?

Community Care is a non-profit organization assisting seniors and adults with disabilities to remain independent in their own homes and communities.

Our Mission

Community Care Northumberland, Colborne provides community based home support services primarily to persons with dependencies related to aging, physical disbility and to their caregivers so that the quality of life of those who receive these services shall be maintained and enhanced.

NEW!  Northumberland Transportation Initiative (NTI)

This new initiative is operated through Community Care Northumberland and provides transportation to and from Colborne, Dundonald, Castleton, Lakeport, Morganston, Salem, Grafton, Centreton and Cobourg (Cramahe Township/Haldimand Van Route).  Visit www.commcare.ca/links.html  and click the Northumberland Transportation Initiative for more details.

 Meals on Wheels:

Hot, nutritious meals are delivered to your home by a volunteer every Monday, Wednesday, and Friday. The meal includes homemade soup or juice, roll, main course and dessert all for a low cost. Special diets can be accommodated. Pre-order frozen meals to create your own menu that can be heated and eaten at your convenience.

Transportation Services:

Volunteer Drivers provide local or long distance transportation for medical appointments, shopping or social activities. There is a set minimal fee depending on the distance. The fee is paid directly to the volunteer driver to cover out of pocket expenses.

Friendly Visitors:

A volunteer will visit in the home on a regular basis to share activities and companionship.

Respite-Caregiver Support:

Provides support for the caregiver, to relieve stress placed on family relationships through one on one visits and educational opportunities.

Diner's Club:

Join with others to enjoy a nutritious meal with an opportunity to hear guest speakers or entertainment. Lunches are held at the Keeler Centre on the 4th Thursday of each month. $10.00 per person.

Caring Callers:

Sometimes it's nice to hear a familiar, friendly voice. A volunteer will call you on a regular basis to find out how you are doing and if you need any assistance.

Home Help and Home Maintenance:

Contracted workers provide assistance for routine household tasks, such as light housekeeping, shopping, meal preparation, snow shoveling, gardening, washing windows etc. A minimal fee for this service is paid to the Brokered Worker.

Information and Referral:

Staff are trained and knowledgeable in regards to other community resources and we are available to assist you. This is a free and confidential service.

Income Tax and Completion with Government Forms

Income Tax services provided by a Certified General Accountant for a nominal fee. Staff will assist with the completion of various Government forms, etc.

Funding

Funding to operate our programs comes primarily from the Ministry of Health - Long Term Care, service clubs, community organizations, municipalities, and local fundraising efforts. We rely heavily on the generosity of individuals to make sure our programs continue to be offered in the community.
