*Community Diners – A monthly program that offers a tasty, nutritious, affordable meal enjoyed with others. It’s a time for fun, entertainment and learning that keeps you connected with your community.
Friendly Visiting – A visit or telephone call for a caring volunteer on a regular basis to share in activities and companionship.
Home at Last- A one –time settlement home from the hospital that can include transportation, grocery shopping, medication pick –up, preparing a small meal and providing personal care and/or homemaking. Qualifying clients may receive enhanced supports to make the transition from hospital to home a success.
*Home Help - Assistance with household activities such as light housekeeping, shopping, and meal preparation. This service is provided by brokered workers.
*Home Maintenance - Assistance with heavier chores such as snow shovelling and yard maintenance. This service is provided by brokered workers.
Hospice Palliative Care – Support is provided to people of all ages who have a life threatening illness or need end of life care. Family and friends are also supported by trained volunteers during these difficult times. Individual grief support and grief groups are available.
Information and Referral:
Staff are trained and knowledgeable in regards to other community resources and we are available to assist you. This is a free and confidential service.
Income Tax assistance:
Income Tax services provided by a Certified General Accountant for a nominal fee.
*Meals on Wheels - Nutritious hot and frozen meals delivered to the homes of adults who are aging, have special needs or are recovering from illness or injury.
Supports for Caregivers -Assistance in meeting the individual needs of caregivers with their on-going responsibilities associated with caring for a loved one, a friend or a neighbour.
Telephone Security Checks – A short telephone call at a pre-arranged time will provide reassurance, a sense of security or send assistance when needed.
*Volunteer Transportation Program- provides transportation for medical appointments, shopping and to various social activities and programs. Local and long distance destinations are available. Volunteer drivers provide the transportation in their own vehicles.
*Specialized Van Service – provides accessible and affordable transportation in both rural and urban areas. Local and long distance trips can be accommodated for both individual and groups.
- *Northumberland Transportation Initiative (NTI)
Provides transportation to and from Colborne, Dundonald, Castleton, Lakeport, Morganston, Salem, Grafton, Centreton and Cobourg (Cramahe Township/Alnwick/ Haldimand Van Routes available servicing Rosneath and Fenella)

Wellness Programs – over-all health and wellness of clients is promoted through a variety of activities and programs such as fitness, blood pressure clinics, choral groups, support groups and more.
Funding to operate our programs comes primarily from the Ministry of Health - Long Term Care, service clubs, community organizations, municipalities, and local fundraising efforts. We rely heavily on the generosity of individuals to make sure our programs continue to be offered in the community.
